An annotation can be created in three ways: with the button 
on the detail page of a single road or segment, via the checkboxes and the Annotate … roads button in the table, or via a drawn line on the map. The window uses the same grouped type selection as the other annotation screens. Choose the appropriate group and type; the available values follow from that type. In the window you fill in:
